How to Diagnose an Alzheimer Disease?- Juniper Publishers


Alzheimer’s disease is a neurodegenerative disorder characterized by an insidious onset, a gradual decline in cognitive functions, a constant but frequent occurrence of non-cognitive disturbances (psychological and behavioral disorders) and inexorably leading to a loss of functional autonomy. Alzheimer’s dementia corresponds to the stage of the illness during which the clinical symptoms (affecting memory and other areas) are sufficient to alter the activities of everyday life. Typical Alzheimer’s disease (common form) is characterized by the installation of an early and significant progressive deficit of the episodic memory that will always remain dominant. Behavioral and psychological symptoms of dementia represent common clinical features of dementias, contributing to the heterogeneous phenotypic expression of Alzheimer’s disease. Recently proposed diagnostic criteria, in addition to the clinical neuropsychological examination aimed at identifying the typical AD symptoms, include staging criteria based on AD biological measures related to its pathology.

Bell's Palsy: Current Status-Juniper Publishers


Bell's palsy, an acute ipsilateral peripheral facial weakness equally affect both sexes, both side of the face and its incidence is 20 people per lakh of population, lowest <10 years age and highest >70 years. Female during 3rd month of pregnancy is more prone as HSV susceptibility is more during the period while children are more susceptible during winter. In life time 1 in 65 persons suffers with Bell's palsy and 7% patient may have recurrence in mean gap of 9-10 years of first episode [1-6].
Normally Bell’s palsy start to regain its motor function of its own within 3 weeks but commonly fails to recover completely and presents with sequel like- Chronic loss of taste (Ageusia), Chronic facial spasm, Facial pain, Corneal injury, Synkinesis, Tinitus and gestational sweating. In addition abnormal nerve regeneration may present with-Crocodile tear (Lacrimation of ipsilateral eye during chewing) and jaw winking (Closure of the ipsilateral eye lid on opening the jaw) [7,8].
The medical literature affirm that 33% patient present with incomplete recovery and associated sequel. The commonly used therapeutic constitutes Antiviral, steroid, neurovitamin andphysical therapy but fails to achieve complete cure and presents with sequel even after 6 months of therapy [9,10].

Dementia is roughly classified into vascular dementia and Alzheimer disease, and the rate of dementia varies according to race or gender [1]. Asians are more prevalent in vascular dementia than westerners and women have a higher prevalence of Alzheimer dementia than men [2].
Aging of neuronal cells can cause Alzheimer dementia, which can lead to daily life by totally relying on the help of others by lowering human mental ability and intellectual function [3]. In addition, modern people are exposed to vascular dementia easily due to severe workload and mental stress caused by high industrialization, resulting in decreased brain function and cerebrovascular disorder [4-6]. A common feature of Alzheimer dementia or vascular dementia is that it causes neuronal death or cell degeneration. In particular, Alzheimer dementia is characterized by pathologic features such as neurofibrillary tangles or senile plaques due to P-amyloid proteins [7].

Parkinson's disease is a degenerative condition of the central nervous system, which majorly affects the motor system. The condition progresses slowly over time causing symptoms like tremors, rigidity, slowness of movement and difficulty in walking in the beginning and dementia follows along with these symptoms in the advanced stages as the disease progresses. There are four cardinal motor symptoms in Parkinson's disease, which are tremor, rigidity, bradykinesia and gait instability. There is no cure for Parkinson's disease but certain treatments are available which slows down the progression of the disease by countering the symptoms [1]. There is also surgical management of implanting electrode for deep brain stimulation, which reduce these motor symptoms. However the underlying cause of Parkinson's disease is not well understood.

In Japan, three medicines are available as a cholinesterase inhibitor for mild to moderate stage in Alzheimer's type dementia, i.e. donepezil, galantamine and rivastigmine. In Japan, donepezil was allowed in 1999 and other two medicines were allowed in 2011. Therefore, main separation of these three medicines is those between donepezil and (rivastigmine or galantamine). In this article we note the separation between donepezil and rivastigmine.
There is no convention and rule for using these three medicines for different purposes. Of course no separation between donepezil and rivastigmine. In fact, physicians are not conscious about the differentiation of these three medicines. It is only emphasized that rivastigmine is available as patch type and two medicines are available as oral administration type. Therefore, in the situation of prescription of rivastigmine, it is emphasized that we should prescribe rivastigmine when the patient refuse to take medicine. It is also emphasized that in order to be calm, it is important for patients and their caregivers, we should provide the opportunity providing them skin ship (direct contact between them and their caregivers thought facial shins)".

His name was Ronnie. His parents called him Ronnie Runabout and nicknamed him "Run Ronnie Run.” They told him that he ran before he walked. At the age of 11 months, he was running in the house, and they couldn’t stop him to even teach him how to walk.
Ronnie was a lovely little boy who was adopted by a childless couple, after the tragic death of his parents in a house fire. He was told that his ancestors were Native Americans from New England. With immigrants settling to the New World, his ancestor’s tribe, lost its identity and distinct culture. Ronnie adoptive parents were Anglo-Saxon Protestants with a sense of pride, honor and valor. They were established and settled in the foothills of the North-Eastern Mountains. They managed a ski resort part time; and the rest of the time operated a quaint bed and breakfast accommodation. Ronnie became the joy and the center of his parents’ life. Due to his unmatched ability to run, his parents thought that he would become a famous athlete and one day competes in the Olympics as a runner. Throughout his school years, Ronnie competed in many races and won them all. He had a dream that one day he would run and win the Olympic Marathon.

A Case Report of Traumatic Temporal Cephalocele- Juniper Publishers



A 40-year-old male patient, hit by a car two years ago, evolved with hypoacusis (hearing impairment) and tissue projecting into the left external auditory canal, detected by otoscopic examination.
A computed tomography revealed an oblique fracture of the left temporal bone associated with hypoattenuation of the adjacent cerebral parenchyma compatible with parenchymal contusion. Bone discontinuity of the tympanic tegmen with the presence of hypo attenuating material in the tympanic cavity and in the homolateral mastoid cells was also observed, this being non-specific to the method.
Subsequently submitted to a magnetic resonance of the skull, meningoencephalic content was distinguished within the tympanic cavity from the middle cranial fossa/tympanic tegmen, representing post-traumatic cephalocele.

Parkinson disease is neuro-degenerative movement disorder which progress slowly over time and affects a person's activity of daily living. The cause and cure has not been found yet. There are some treatments available, which can slow down the progression of the disease by countering the symptoms. Long-term use of medications results in involuntary movements, which becomes a problem for the person as well as for the clinician as the medication cannot be taken off. In the recent times a new rehabilitation discipline known as Neurologic Music Therapy has shown significant improvement in the Parkinson's disease symptoms. This paper provides a review of Parkinson's disease and the Neurologic Music Therapy techniques used in Parkinson's disease. The paper also attempts to provide the management of medication induced involuntary movements in Parkinson's disease by using Neurologic Music Therapy.

With the many recent advances in the pharmacological treatment of psychiatric conditions.The pharmaceutical industry has been vigorous in promoting their products as being very effective intervention for the treatment of mental and psychiatric conditions. Health insurance companies have also been very restrictive in reimbursing psychiatrists for their clinical services if they do not prescribe medications for their patients' treatment [1]. As a result, many psychiatrists have been pressured to limit their scope of practice to prescribing medications and have been labeled as prescribers as opposed to being physicians who specialize in providing comprehensive and holistic mental health treatment to patients who suffer from psychiatric conditions [2]. The purpose of this commentary is to redefine the role of psychiatrists not just as prescribers but as providers of their time honored calling as holistic healer of mental illness.

Helping Children Understand Social Shades of Gray; (TBSND)-Juniper Publishers








One of the many challenges of growing up is contained within the sometimes mysterious world of peer culture: misunderstood meanings of social behavior. The emphasis on dichotomizing peer behavior as either good or bad, reinforced by the broad definitions of bullying, has planted the seeds for many children to label peers as either nice or mean. This sets the stage for personalization, avoidance, accusation, and other social traps that are the result of skewed perceptions. Difficulty understanding social shades of gray can be resolved when children learn how to consider that the vast majority of peer behavior is not nice nor mean, but in-between.
