Intrauterine Insemination with Fresh Versus Cryopreserved Spermatazoa in Unexplained Infertility

The purpose of this study was to
compare pregnancy rates and live birth rates in women with unexplained
infertility undergoing ovulation induction and intrauterine insemination (IUI)
using fresh versus cryopreserved semen at two institutions. Retrospective
cohort study conducted at two tertiary care centers via chart review of all
women with unexplained infertility who underwent IUI. Other etiologies of
infertility were excluded. Pregnancy, clinical pregnancy, and live birth rates
were calculated in both groups. Odds ratios and 95 % confidence interval were
calculated. A P-value was considered significant when less than 0.05. A total
of 566 IUI cycles were analyzed; 513cycles (90.64%) used fresh semen while 59
cycles (10.42%) used cryopreserved. Clinical pregnancy rates were 29.24% in
IUI-fresh and 11.32% in IUI-cryopreserved (OR 3.24, 95% CI 1.36-7.73, p-value
0.0055). The live birth rate per cycle for IUI-fresh was 19.88% versus 3.77% in
IUI-cryopreserved (OR 6.33, 95% CI 1.52-26.43, p-value 0.0044). Significantly
higher pregnancy rates and live birth rates were observed when using fresh
semen compared to cryopreserved semen in women with unexplained infertility
undergoing ovulation induction and intrauterine inseminations.
