Leiomyoma is a benign smooth muscle tumor of metastatic potential. In this paper we present the second reported case of leiomyoma in the middle ear cleft in a 59-year old Saudi lady who presented with complaint of chronic left ear discharge for 15 years. Computed tomography scan revealed soft tissue density with calcification in the epitympanum and mastoid antrum without bony erosion. The mass was successfully removed by canal wall up mastoidectomy approach.Theoriginof leiomyoma is likely to be the smooth muscle of the vasculature of a middle ear cleft, which is devoid of other smooth muscles.
